GREENVILLE, S.C. - SCaroNews -- ChartSpan, a value-based care organization, has expanded their preventive care offerings with a full-service Advanced Primary Care Management program (APCM). ChartSpan APCM will now be available to all providers who serve as the central point of care for their patients.
APCM offers Medicare patients preventative care between appointments, with the aim of achieving better health outcomes and reducing healthcare costs. It differs from previous Medicare care management programs because of its availability to all patients and absence of time requirements.
Any patient who has Medicare is eligible to enroll in APCM, regardless of how many chronic conditions they have. There is also no set amount of time providers and care managers must spend with patients each month. Instead, providers can bill APCM any month that they offer APCM services to enrolled patients.
